American higher education is one of the oldest in the world. Since the establishment of the first higher institute of education in the country, there has been tremendous growth in the number of students admitted to our higher education institutions every year.In deed, College education has been the aspiration of the many young adults in the country.   As the modern technology takes centre state in life, education has become in important tool to assist a student to cut an edge in the technology driven economy.However the issue of admission to American colleges has remained controversial in respect to the admission of average or mediocre students. While some have argued that the admission of average and mediocre students has debased American higher education, this paper strongly refutes this claim. However some of the average or mediocre students have performed better than other students. Therefore the first point that refutes this claim is that our system has poorly divided and graded our students (Thill, 2006).SAT has been widely used in determining the eligibility of a student to higher education institutions.   There are many scholars who argued against the use of SAT to determine the eligibility of student to higher education arguing that it is flawed and very unreliable.The first factor that has debased American higher education is the use of SAT in determining the eligibility of students. Therefore we can say that there are no averages or mediocre students but we only have a mediocre system that is used to scrutinize them and this system is SATs.There are other college admission criteria that have debased American higher education system. There are some students who are admitted to college based on their past sports record. Rothchild (2008) argue that there are a number of top colleges which pay students exorbitant amounts of money or award with possessions in order to sway students to join their colleges and play for their teams.No one is quite sure whether the coaches who admit these students to their team really look into the minds like the SAT test looks into student minds.   Most of these students do not really deserve to be college student as their mediocre or average classmate.This means that it is not the admission of average and mediocre students which has debased American higher students but rather the discriminate admission of students based on other factor like sports career.

Music is said to be around since the dawn of time and it has become an integral part of human life. It took different forms since its evolution and from drums to flutes and harps it became better and numerous instruments were added like piano and guitars. The band was formed in February 1968 by Chris Curtis who was an ex-drummer of the band called The Searchers. At that time The Searchers was quite popular among the crowd and was said to be the rival of the great The Beatles but when Curtis left the band he formed a small band with some session players and had a hit called â€Å"Let’s Go to San Francisco† after which he decided to have a proper band .The band has gone through a lot of twists and turns with some of the members leaving the band and rejoining it later, some just joining the band for a short period of time and then leaving it while a few of them died. Deep Purple initially called â€Å"Roundabout† had the first line up of members with Ritchie Blackmore as guitarist, Chris Curtis and Dave Curtis for vocals, Jon Douglas Lord as pianist and on keyboards, Nick Simper as bassist and Bobby Woodman as drummer.But soon Cur tis left the band with his brother but Blackmore and Lord were enthusiastic to carry on the band and so Curtis was replaced by Rod Evans on vocals and when Curtis left Booby Woodman left the band too based on the fact that it was the experimental situation they were going through so Ian Paice came as the drummer for the band and is said to be the only original member who did not leave the band up till now. Therefore the â€Å"Roundabout† after their small tour of Denmark in 1968 changed the name of band to â€Å"Deep Purple† on Ritchie Blackmore’s suggestion.It is said that Blackmore’s grandmother loved the song deep purple on which he decided to credit her and this line up of band is said to be the Mark I line up and this band has went through almost VIII line ups. The Mark I line up started to gain fame after doing a cover of Hush a song by Joe South and it got 4th place on the US Billboard chart, the song was from their fist album â€Å"Shades of Deep Purple† released in 1968 after which they were signed up for some tours too.The band launched their second album â€Å"The book of Taliesyn† and it reached #38 on the US Billboard chart while the third album â€Å"Deep Purple† but due to bankrupting of the American record company Tetragrammaton and numerous tours, the band got left hanging on a thread with no future possibility and financial assets. Ultimately the group members were left in confusion resulting in Rod Evans and Nick Simper leaving the band after they were fired due to some internal matters.This resulted in the Mark II line up when vocalist and bassist were required by the band. After some searching and auditioning Ian Gillan was selected as the lead vocalist of the band with Roger Glover replacing Simper as bass guitarist. Gillan then went out of the music scene and started a motorcycle manufacturing company and some years later formed his own band with his name while Glover carried out what he did best so after the two left the band the vacancy was to be filled thus auditions were carried out and vocalist David Coverdale and bassist cum vocalist Glenn Hughes were signed in.Coverdale used to play guitars but decided to sing after some support from the people while Glenn Hughes was known for his notable performance in â€Å"Trapeze†. This group or Mark III then launched their first album and overall eighth album titled â€Å"Burn† in February 1974, this album is also said to be the best comeback of the band as both the new members did their jobs in a brilliant and fruitful way producing a better output that the band required to get back in shape.The songs that made the spot light were â€Å"Might Just Take Your Life† and â€Å"Burn†( Thompson ,90). The second album was released named â€Å"Stormbringer† in 1974 but as this album had funky playback, although the album had many popular songs like â€Å"The Gypsy† and â€Å"Lady Double Dealer† guitarist Ritchie Blackmore again showed desperation and left the band in 1975 to form his own band Rainbow, stating that he was not into funky soul music.The departure of Ritchie Blackmore was the greatest set back for the band as his position was one of the most important and no other guitarist was available who could perform like him, but the real shock to the fans came when the band decided to carry on replacing the irreplaceable Blackmo re with Tommy Bolin who was an American famous for many musical performances making this a forth line up better known as Mark IV line up.So the band than released the album entitled â€Å"Come Taste the Band† in October 1975, the album was a hit but again the problems arrived with the band’s future when Bolin’s drug addiction interfered the bands performance as due to his condition many concerts and low scale appearances were being cancelled. The Mark IV only lasted a year after Bolin was found dead due to drug overdose while after the Britain’s tour in March 1976 Coverdale quit and the band officially announced the split in July 1976(Prato,35).The band was then split up for almost eight years and during this time each member started some side projects concentrating on them but after an eight year separation the band made a return with the Mark II line up in April 1984 and released an album â€Å"Perfect Strangers† in October 1984 and this time it was a massive hit reaching #6 on US Billboard 200. The band’s tour was initiated worldwide in Australia, North America and Europe which made the band financially better too.â€Å"The House of Blue Light† was the band’s twelfth album launched in 1987 but in 1989 Gillan was dismissed as the rivalry between him and Blackmore grew therefore he was replaced by vocalist Joe Lynn Turner who belonged to the band Rainbow of Blackmore. The Mark V line up than recorded only one album â€Å"Slaves & Masters† but this was not given a positive response by the fans as they thought it was more Rainbow than Deep Purple.So after the Mark V made a tour Turner was removed by the record company to bring Gillan back for the bands 25th anniversary, resistance was made by Blackmore but after some cash transfer he agreed and the Mark II line up again released the album â€Å"The Battle Rages On† which is the band’s fourteenth album however as the rivalry of Gillan a nd Blackmore was still on so Blackmore left the band for good in November 1993.Thus the band’s Mark VI line up came when the famous Joe Satriani replaced Blackmore but due to his own record company contracts he could not carry out the job permanently and therefore he left in 1994 after completing and helping he band with the tours. The band then drafted Steve Morse to take Blackmore’s position permanently and he is still the band’s member today. This was the Mark VII line up of Deep Purple and this line up was carried out till 2002 during which the band released the albums Perpendicular in 1996 and Abandon in 1998.In 2002 the band’s oldest and pioneer member Jon Lord left due to some personal reasons and his replacement was Don Airey who was a former member of Rainbow thus the band’s final Mark VIII line up came into being and almost two more albums were released named â€Å"Bananas† and â€Å"Rapture of the Deep†. Buddhism is gaining popularity and acceptance with the American public. This could be credited to the immigrants to the USA wherein some of them brought with them their culture and religion and influenced the Americans to follow them. Even though Americans make up less than 5 percent of the worlds population, we consume close to 100% of the worlds supply of hydrocodone and nearly 80% of the worlds supply of legal and illegal opiates . Because of the vast availability of drugs and its impact on many facets of healthcare and the economy, it is imperative that health care providers and policy makers understand what drugs are being abused, who uses these drugs, where the drugs come from, and the health and economic burden on the United States. It is becoming exceedingly important for those directly involved with substance abusers to understand the physiological consequences, psychosocial consequences and withdrawal consequences of drugs. Opiates and opiate derivatives in particular, whether illicit (such as heroin) or prescription (such as Oxycontin) are being heavily abused across the country. These drugs can have serious health implications during use and during abrupt cessation (detoxification or withdrawal). This paper will present a review of the literature on the epidemic of heroin and prescription opiate abuse. Through a critical review, concepts such as the prevalence of the problem, the physiologic consequences of abuse, the current trends in management of the problem and implications for nursing and healthcare will be explored. Background Before proceeding, it is important to briefly discuss the basic pharmacology of opiate drugs. Opiates, otherwise known as narcotics, include the drugs heroin, morphine, codeine, oxycontin, hydrocodone, methadone and other chemically-related derivatives. All opiates are considered dispensable only by prescription in the United States with the exception of heroin, which is illegal in all circumstances. Opiates wield their effects by activating pleasure centers in the brain. The neurobiochemical model of addiction suggests that over time, the brains chemistry changes so that it yearns for the substance when it is not there and can produce physiologic effects of withdrawal when stopped. Heroin for example, which is chemically-related to morphine (the chemical name for heroin is diamorphine) readily crosses the blood-brain barrier to cause a rapid spilling of dopamine into the dopaminergic receptors of the brains neurons. For this reason, the drug is easily addictive and absence of the dr ug causes a sensation of craving and can cause feelings of illness, such as nausea and diarrhea, when stopped abruptly. . Prevalence of the Problem Several researchers in the nursing, allied health and economics disciplines have explored and discussed the prevalence of prescription opiate abuse and hypotheses of the origin of the problem. However, there is a large gap in the literature related to the prevalence of illicit drug abuse (heroin) among different populations. The prevalence data for heroin abuse in the US is largely reported by national survey trends, such as the National Institute of Drug Abuse (part of the National Institutes of Health) Survey on Drug Use and Health (NSDUH). Even so, the massive survey document pays little attention to the problem of heroin abuse and reports only two percent of the 67,500 survey respondents using heroin in the month preceding the survey (National Survey on Drug Use and Health, 2009). The etiology of prescription drug misuse and abuse is discussed more heavily in the literature than is heroin abuse. According to the NSDUH, in 2009 4.8% of the survey respondents aged 12 and older reported using prescription opiates for non-medical reasons. Of those, over 55% reported obtaining the drug for free from a friend or relative, and 4.8% reported purchasing the drug from a dealer on the streets (National Survey on Drug Use and Health, 2009). Researchers at Columbia University surveyed adolescents aged 12 to 17 in 2005 and found that 9.3% reported the misuse of prescription opiates over the past month. In exploring the adolescents motives for abuse, the researchers found that the most commonly reported motives were ease of attainment and affordability . Among young adult college students, prevalence of non-medical prescription opiate abuse has been reported between seven and nine percent in the past month to twelve to fourteen percent lifetime prevalence . One of the most at-risk groups for misuse of and addiction to prescription opiates is the chronic pain population. When used appropriately, opiate pain medications are a gold standard of care for serious pain-related conditions (such as post-operative pain, neuropathic pain and so on). In an effort to better manage pain, the number of prescriptions for opiate medications increased 154% in the ten-year period from 1992 to 2002, even though the population of the United States increased by only 13% . Although prescription opiates have been used to treat pain and improve the quality of life among acute and chronic pain patients, the advent of the drugs misuse has led to a wide body of literature on prescriptive practices and monitoring of opiate drugs. There delicate balance between managing pain adequately and the risk for misuse, addiction and diversion has been discussed in the literature recently, largely among the adolescent/young adult and chronic pain populations. Although there is a general lack in the literature of the number of people prescribed opiates for chronic pain . There is a large body of literature which suggests that the increased number of prescription opiates for chronic pain patients has contributed significantly to the drug abuse problem. In studies to investigate the use of prescription opiates for chronic pain, it has been found that pain treatment plans are sparse, good documentation on the patients health history and history of substance abuse is rare, and opiates are sometimes improperly utilized or not indicated at all . Health Consequences of Opiate Abuse Both prescription and illicit opiates have the potential to exert a myriad of acute and chronic negative health effects for the user. Neurological decline, vision changes, cardiac abnormalities, vascular complications, pulmonary disorders and mortality have been studied and reported. In a study of over 500 opiate addicts, electrocardiographic changes were documented in 61% of the sample. These ECG changes included ST-segment elevation, which may indicate ischemic heart damage (or lack of oxygen-rich blood to the myocardium) and QTc prolongation (seen mostly with methadone addicts). Prolonged QTc intervals may lead to inappropriate triggering of the ventricles and may lead to potentially lethal ventricular tachycardia . The use of heroin and other drugs by injection with non-sterile or reused needles may produce powerful toxins in the bloodstream leading to endocarditis, left ventricular hemorrhage and death. Researchers in Ireland reviewed the autopsy findings from eight cases of heroin users and found rapid fatal illness caused by the endotoxin Clostridium novyi which caused fatal subendocardial hemorrhage, spleen enlargement and pulmonary edema in all of the subjects studied . Heroin can cause significant acute peripheral and central nervous system complications such as neuropathy and hearing loss. In a study six of intravenous and intranasal heroin users admitted to the hospital for acute illness, five had documented rhabdomyolysis between three and 36 hours after administration of the drug. In each of the five cases, there was no documented trauma. Typically rhabdomyolysis occurs after a traumatic, compression-type injury where enzymes spill into the musculature and cause rapid neurological decline, loss of muscle tone and coma. In each of the subjects studied, no trauma or compression injury occurred . In a group of patients with a reported history of speedballing (or mixing heroin and cocaine), 2 of the 16 studied experienced sudden, bilateral sensorineural hearing loss four hours after speedballing which resolved within three days. One additional participant experienced sudden hearing loss after speedballing but unilaterally. The mechanism by which th e hearing loss occurred was described as a possible cochlear toxicity or autoimmune reaction related directly to the drugs . In the most severe cases, opiate abuse may lead to death, generally from overdose. Heroin has been related to 1.65 deaths per 100,000 people in the Florida population alone over the last decade and in Alabama between 1986 and 2003, a review of medical examiners reports revealed those whos cause of sudden death was undermined were 5.3 times more likely to have a history of drug abuse . Researchers in the United Kingdom have further investigated heroin deaths and concluded that overdose related to heroin rarely occurs after the use of heroin alone- in 50% of the cases they reviewed, alcohol was also involved . Although studies have been published on the health effects of abuse itself, a review of nursing, medical and allied health literature shows a significant lack of studies on the health consequences of withdrawal (abrupt cessation) of opiate drugs. Several studies have been found relating to the use of pharmacotherapy to treat opiate addiction (such as methadone and similar programs) but no studies have explored the physiologic effects of withdrawal without medication assistance. Searching online databases for relevant articles on opiate withdrawal does not yield any studies other than those using medication-assisted withdrawal therapies (such as methadone and buprenorphine). This is important to populations such as inmates in a correctional facility where pharmacotherapy is not generally used during detoxification and requires further attention . In sum, the potential health-related consequences of opiate abuse can negatively affect a myriad of body systems. The risk of potentially fatal infectious disease, neurological and cardiovascular complications and death by overdose creates a public health problem that demands attention. Health care providers in all arenas must be able to rapidly and appropriately identify those at risk. Implications for Advanced Nursing Practice Nurse practitioners are allowed prescriptive authority in almost any state across the country. Depending on the practice setting, they may prescribe opiates to treat pain. Opiates are the gold standard for treating moderate to severe pain in both acute and chronic pain syndromes . There has been some discourse in the literature on the dilemma to treat or not to treat pain with opiates. On the one hand, effective pain management is not only ethical but also opiates are effective for many patients for pain management and on the other hand there is a fear of misuse, dependence and addiction issues . The ability for advanced practice nurses to identify drug-seeking behavior, issues with addiction and possible diversion of opiates is crucial to the problem of opiate abuse. As it has been documented in the literature, diversion of prescription opiates generally occurs with the patient selling, trading or giving away the medication to another. According to Annie Gerhardt, emergency room nu rse practitioner, drug seekers become victims of their own disease, becoming involved in a tangled web of addiction, drug-seeking, drug diversion and criminal behavior that escalates over time with increased drug use . Gerhardt (2004) suggests monitoring patients treated with opiates for refilling meds before their follow up appointments, patient reports of losing medications, attempting to see different providers for pain medications (which can usually be identified by pharmacies), and scamming providers for more medications by complimenting the provider or the office staff or displaying increased levels of pain in order to procure more medication. When prescribing opiates to treat pain in patients already known to have addiction issues, using sustained-released formulations may reduce dependency while adequately treating pain. Sustained-release formulations usually have a reduced street value and are less likely to be sold and diverted, and the withdrawal from sustained-release op iates is typically not as severe . In terms of advanced practice nursing, prescribers have the unique ability to screen for potential addiction issues, monitor those who are prescribed addictive medications and potentially control diversion of those medications to others for whom they are not prescribed. Ultimately, the knowledge of drugs of abuse and health-related consequences is important for nurses at all levels to possess. Implications for Research To further add to the body of literature on the subject of opiate drug abuse, addiction and withdrawal, several areas still need to be explored. First, patterns of drug use in individual populations and communities should be investigated. The types of drugs most commonly used, the routes of administration of these drugs and their negative health-related consequences should be studied in a variety of communities, both urban and rural, to gain a better understanding of the patterns of use across the country. More research is needed beyond simple prevalence studies on the number of people who use to expand on the actual patterns of abuse. Similarly, the psychosocial consequence of drug use across different populations needs further investigation in order to articulate the negative impact on individuals and communities. Because the abrupt cessation of opiate drugs without medication assistance occurs frequently in specific settings, more research needs to be done on the physiological effects of non-assisted detoxification. The patterns of symptoms of withdrawal and negative health effects among populations such as the incarcerated and the hospitalized need to be explored. The potential for the development of evidence-based protocols for the management of abuse and withdrawal in these settings can occur once the patterns of symptoms and health consequences are documented. Conclusion In sum, the problem of drug abuse (which extends far beyond even opiate drugs) has been reported in multiple sources, and prevalence studies seem rampant in the literature. However, much more work needs to be done on the consequences of drug abuse from a healthcare perspective across a variety of populations in different settings to begin to understand the impact of drugs on communities.

Forced to move on, Palestinians traveled to countries like Jordan and Egypt in search of shelter, which in turn stimulated the conflict from a Palestinian one to a Middle Eastern one. â€Å"TheIsraeli-Palestinianconflictis actually a hyper conflict influenced by local, regional, and global factors. It is no accident that the main accords producing temporary stability have been reached in periods of relative calm for the whole Middle East† (Casertano). The Palestinian conflict is in reality a Middle Eastern conflict, while the gruesome war is fought on the grounds of Jerusalem, Palestine, the soldiers are equipped and assembled in Sinai, Egypt and trained by the most elite sergeants in the Jordanian Army and Syrian Navy. Adult males, children and even females all join the fight against the oppressing Israelis through any means possible. Adult Males try to attack Israeli soldiers using anything from Ak-47’s to rocks and glass bottles; females on the other hand find themselves in varying roles. In Palestine, females join Hamas and serve in a plethora of jobs, jobs that range from nursing and helping the wounded, to holding a weapon and attacking the nemesis. And then there are the females who discover that the only way to freedom is through perversity like Ayat al-Akhras who â€Å"Pushed a button on the belt strapped to her and detonated an explosive device in a crowded supermarket in Kiryat HaYovel, Jerusalem† (Giovanni). For Syria, Palestine’s close neighbors, females recruited by Al-Nusra, â€Å"a branch of Al-Qaeda creat ed in January 2012 that operates in Syria and Lebanon. Which has also been cited as the most aggressive and successful of the Syrian rebel groups† serve in deadly roles, transporting bombs across the border and fighting on the frontlines (Giovanni). The Israeli state deserves to exist, but viewing a group like Hamas, fighting for the survival of its torn nation, as a deadly terrorist organization is at the least contradictory considering that Americans live in a country amassed through a revolution. While groups like Hamas should be viewed with a little more sympathy, groups like ISIS should be viewed with more animosity and revulsion. Islam is similar to Christianity in a way; both have factions in which religion is interpreted differently and whether that difference be a mild or extreme one, the factions identify themselves as separate sectors of the religion. Comparable to the Protestant/Catholic conflict during the times of the Holy Roman Empire, the Sunni/Shiite conflict today remains at intense levels in the Iraqi homeland. Sunnis have always been at odds with Shiites; but nowhere have they pugnaciously acted out against each other like they have in Iraq. This violent relationship has caused the creation of groups like the Islamic State or ISIS. However, ISIS did not find its ignition in Iraq, rather it found its true base formed in the revolutionary sands of Syria. While George Bush was signing the agreement that ended the American Iraqi War, ISIS was moving out of Iraq and into Syria. Joining the Syrians in their fight against the government of President Bashar Assad, ISIS gained a stronghold in that area a fter losing the stronghold they had in Iraq to the American troops during Operation Iraqi Freedom (â€Å"Who’s To Blame†). The group made a profit, detained armor and weaponry, recruited followers and â€Å"became battle-trained and hardened† through its fight against Assad (â€Å"Who’s To Blame†). Quickly, this weak organization created an arduous force and refurbished from a group known as Al Qaeda in Iraq, to a terrorist organization feared and known as ISIS. â€Å"What began as a small group of individuals with a relatively common extremist ideology has grown to become one of the largest and most barbaric terrorist organizations in the world† (O’Briain). Today, Most Americans know about ISIS because of its intelligent advertising techniques. ISIS has managed to recruit talented individuals who were able to transform the media and technology fields to a whole new level; attracting more recruits and producing an outburst that has caused television to unintentionally promote the terrorist organization. The interesting thing about ISIS is the way it runs its â€Å"government†. They have taken away the Iraqi populations original government issued ID card and assigned them a new ISIS ID card to replace it (Cambanis and Collard). Furthermore, ISIS has redrawn the Middle Eastern border and legitimately created an ISIS nation. Unfortunately, for them, no other country in the world recognizes this border; but at the rate ISIS is developing today, the world might have to recognize the Islamic state’s redrawn border shortly. â€Å"ISIS looks much more like a functioning government than any of its detractors ever thought it would: it is pumping oil, policing streets, collecting taxes, [and] even planning to issue its own currency† (Cambanis and Collard). Although the Islamic State has assembled an organized and sophisticated ruling system, the way it has structured its judicial court and laws are on the complete opposite spectrum of civilization. The Islamic State runs an extremely strict code of law based on a faulty interpretation of The Quran, the Muslim holy book, and has caused great distress to those living within the makeshift border.
